There are few things more frustrating than finally seeing your skin clear up… only to wake up a few weeks later with another breakout. It can feel like you're back at square one.

Many people start questioning everything. Was it something I ate? Did I use the wrong cleanser? Are my hormones suddenly out of balance again?

The reality is that acne is rarely a straight line. Most people experience periods where their skin improves, followed by times when it flares again. Understanding why this happens is often the first step toward finally breaking the cycle. Let's look at some of the most common reasons.

My Patient - Lily

Lily had almost forgotten what it felt like to enjoy looking in the mirror.

After years of stubborn acne, she'd finally had six weeks of relatively clear skin. The painful cysts had settled down, her makeup went on smoothly, and she felt confident enough to leave the house without foundation.

Then one morning she noticed a deep, sore bump along her jawline. By the end of the week, there were three more. She was devastated. She wondered whether everything she'd been doing had stopped working.

The truth was, nothing had "failed." Her skin was simply responding to changes happening inside her body that she hadn't yet recognized.

Acne Is Dynamic

A big misconceptions about acne is that there's one single cause. In reality, acne develops when several factors overlap:

  • Increased oil production
  • Dead skin cell buildup
  • Acne-causing bacteria
  • Inflammation
  • Hormonal signalling
  • Genetic predisposition

Some of these factors are constantly changing. Your body isn't the same every day, so your skin isn't either. That's why acne often comes and goes instead of remaining exactly the same all year long.

Hormones Naturally Fluctuate

Many women notice predictable breakouts around their menstrual cycle. Even if hormone blood tests are completely normal, monthly hormonal fluctuations can temporarily increase oil production and inflammation.

For others, acne may flare during:

  • Coming off hormonal birth control
  • Times of high stress
  • Pregnancy or postpartum
  • Perimenopause

These hormonal shifts don't necessarily mean something is "wrong"—they're part of normal physiology. However, if your skin is particularly sensitive to these changes, even normal fluctuations can trigger breakouts.

Stress Doesn't Always Cause Immediate Acne

One of the most confusing things about stress-related acne is that the timing rarely lines up.

You might have a stressful week at work, feel completely fine, and then notice a breakout 10–14 days later. Stress influences multiple pathways involved in acne, including inflammation, oil production, sleep quality, food choices, and healing. Sometimes the breakout is delayed enough that it doesn't even seem connected to the stressful event.

That's why keeping track of stress alongside your skin can sometimes reveal patterns you hadn't noticed before.

Sometimes Improvement Is Only Treating the Symptoms

Many conventional topical acne treatments reduce inflammation or suppress breakouts while you're using them. That can make it feel like the acne is gone.

But if the underlying drivers haven't changed, the breakouts often return once the treatment is stopped—or when life throws a new stressor into the mix.

This doesn't mean the treatment was ineffective. It simply means that symptom control and addressing contributing factors are two different goals.

Rather than reacting to each individual pimple, we look for the bigger picture. This approach helps identify recurring triggers, monitor progress over time, and make thoughtful adjustments based on how your skin responds. In my Breakthrough Acne Program, we don't jump straight to supplements or assume every breakout has the same cause. Instead, we work through the major systems that commonly influence acne—including digestion, inflammation, hormones, blood sugar regulation, stress, and skincare—to better understand what may be contributing to your pattern of breakouts.

If your skin clears for a while and then suddenly breaks out again, it doesn't necessarily mean you've gone backwards. More often, it's a sign that your skin is responding to changes happening inside your body or around you.

If you're tired of feeling like you're constantly starting over, you don't have to figure it out alone.

If you're curious about the root causes behind recurring acne, start by watching my free webinar, "3 Reasons Your Acne Isn't Clearing (That Your Doctor Never Told You)." You'll learn why acne often persists despite doing "everything right" and how a root-cause approach differs from simply treating individual breakouts.
