It usually starts with good intentions.

You look at your skin and decide you’re ready to take control. You start researching. You clean up your diet. You cut out sugar. Then coffee. Then gluten. Then anything “processed.” Maybe even carbs.

At first, there’s a sense of progress. You feel disciplined. In control. Like you’ve finally found the reason your skin hasn’t been clearing. And sometimes, there is a short-term improvement.

But then something shifts.

Your skin becomes unpredictable again. Breakouts return — sometimes worse than before. Your energy dips. Cravings increase. You feel more reactive, more stressed, and paradoxically, less in control than when you started.

This is where many people get stuck in a cycle that feels confusing: If I’m doing everything “clean,” why is my acne not improving?

The answer is often not about what you removed — but about what your body experienced in the process of removal.

When people think of “extreme dieting,” they often think of weight loss or calorie restriction. But biologically, the body interprets it much more broadly.

From a physiological perspective, sustained restriction — whether intentional or not — is interpreted as a form of stress.

The body does not differentiate between:

A structured “clean eating plan”
A restrictive elimination diet
Periods of undereating due to food fear or rigidity

It responds to all of them through the same survival framework.

One of the primary responses is activation of the stress system, including the hypothalamic-pituitary-adrenal (HPA) axis. This leads to increased cortisol output, which is meant to help the body adapt in the short term.

However, when this state persists, several downstream effects can occur:

Increased systemic inflammation
Slower skin barrier repair
Altered insulin and blood sugar regulation
Heightened sensitivity of oil glands to hormonal signaling

In other words, the body shifts resources away from long-term repair and toward short-term survival.

The skin, being a non-essential organ in survival terms, is often one of the first places this shift becomes visible.

Elimination diets are not inherently problematic. In many cases, short-term structured elimination can be clinically useful for identifying food sensitivities or reducing inflammatory load.

The issue arises when elimination becomes:

Broad
Long-term
Increasingly restrictive
Driven by fear rather than observation

When too many foods are removed for too long, several unintended consequences can develop:

1. Nutrient gaps

Reducing dietary variety can limit intake of key nutrients involved in skin repair, including zinc, essential fatty acids, protein, and certain antioxidants.

2. Gut microbiome disruption

A diverse microbiome thrives on dietary diversity. Over-restriction can reduce microbial variability, which may indirectly influence inflammatory regulation.

3. Increased food stress

The psychological load of constant vigilance around eating can activate stress pathways that overlap directly with acne physiology.

The key distinction is this: Elimination diets can be useful as a short-term investigative tool, but problematic when they become a long-term identity or default state.

The skin is one of the most metabolically active tissues in the body. It is constantly renewing, repairing, and responding to environmental and internal cues.

That process requires consistent resources.

Key nutrients involved in skin repair include:

Protein (for structural rebuilding)
Zinc (for inflammation regulation and wound healing)
Essential fatty acids (for barrier integrity)
A broad range of micronutrients (for enzymatic and immune function)

When diets become overly restrictive, even unintentionally, the body may prioritize essential organs over skin repair.

This does not mean deficiency is always severe or obvious. It often exists as a functional limitation — where the body has enough to survive, but not enough to optimally repair skin.

This is one reason acne can become slower to heal or more reactive during periods of prolonged restriction.

Extreme dietary approaches often begin with a desire for control. Control over symptoms. Control over uncertainty. Control over skin that feels unpredictable.

But biologically, the skin responds more predictably to something else entirely: stability.

When restriction becomes chronic, the body can interpret it as stress. That stress can influence cortisol, inflammation, and hormone signaling — all of which are directly relevant to acne. This is why some people see initial improvements with strict diets, followed by plateaus or flare-ups later on. It is not a failure of discipline. It is a reflection of physiology adapting to sustained constraint.

If this is resonating, it may also connect to other less obvious drivers of acne that are often overlooked — such as nutrient imbalances from supplementation, or everyday habits that repeatedly disrupt skin healing. These are exactly the kinds of patterns we will continue unpacking in this series.

Because acne is rarely driven by a single factor.

It is usually the result of multiple small inputs that, together, create a system that becomes harder to regulate over time.
