Alexis was 35 and confused. She’d never had acne as a teen, but now, her skin was breaking out regularly. Everyone told her it was "just hormones," but her labs were normal. What was really going on?

If you’re a woman in your 20s, 30s, or even 40s still dealing with breakouts, you’ve probably heard the familiar explanation: “It’s just your hormones.” And while hormonal fluctuations are a major player in adult acne—especially the kind that flares up before your period—they are rarely the whole story.

Adult acne is complex, and the breakouts you see on your skin are often a symptom of something deeper happening within your body. As a naturopathic doctor, my approach is to look beyond the surface and identify these other, less obvious root causes.

Here are three key areas that could be contributing to your adult acne, and what you can do about them.

1. The Gut-Skin Axis: What’s Happening Inside, Shows on the Outside

Your gut and your skin are in constant communication, a two-way street known as the "gut-skin axis." When your gut microbiome—the trillions of bacteria living in your digestive tract—is out of balance, it can trigger a cascade of inflammation throughout your body, including in your skin.

  • The Problem: An imbalance of gut bacteria (known as dysbiosis) can lead to a "leaky gut," where toxins and inflammatory compounds escape into your bloodstream. Your body responds with an inflammatory reaction, which can worsen acne.

2. The Stress Connection: Your Body's Fight-or-Flight Response

Stress doesn't just make you feel anxious; it has a direct and powerful impact on your skin. When you’re stressed, your body releases a hormone called cortisol.

  • The Problem: High cortisol levels tell your sebaceous glands to produce more oil. This excess sebum mixes with dead skin cells and bacteria, creating the perfect storm for a breakout.

3. Dietary Triggers: The Foods That Fuel Inflammation

The link between diet and acne is stronger than ever, with research pointing to a few common culprits that can drive inflammation and breakouts.

  • The Problem: Foods with a high glycemic index (like refined sugars and white bread) cause rapid spikes in blood sugar, which in turn leads to an increase in hormones that stimulate oil production. Dairy, especially skim milk, has also been linked to acne due to the hormones and growth factors it contains.

How to Find Out What’s Driving Your Acne:

Tackling adult acne can feel overwhelming when you're only focused on topical treatments. Before you get discouraged, you can start by playing detective on your own!

Try keeping a simple food and lifestyle journal for a few weeks. Each day, jot down what you ate, how stressed you felt, how much you slept, and if you noticed any changes in your skin. This practice can help you identify patterns and potential triggers you may not have noticed before.

However, a journal can only get you so far. A personalized, holistic plan is often needed to address the root cause of your breakouts. As a naturopathic doctor, I work with you to interpret these patterns and uncover the underlying issues—whether they're related to your gut, stress, diet, or a combination of factors.

If you're tired of guessing games and want a personalized, holistic plan to get clear, confident skin, let's talk.
