When you think about acne, you probably think of clogged pores, oily skin, or maybe hormones like testosterone. But there’s another major player in the acne story that often gets overlooked: cortisol, your main stress hormone.

Let’s break down how cortisol affects your skin—and how you can support your body to bring this hormone back into balance for healthier, clearer skin.

What Is Cortisol, and Why Does It Matter for Acne?

Your body has a built-in stress system called the HPA axis—short for hypothalamic-pituitary-adrenal axis. It’s a communication loop between your brain and adrenal glands that helps you deal with stress. When something stressful happens (like a deadline, lack of sleep, or emotional pressure), your brain tells your adrenals to release cortisol. This is part of your natural “fight or flight” response.

In short bursts, cortisol is helpful. But when it stays high for too long, it can create problems. Chronically elevated cortisol can:

  • Increase inflammation
  • Disrupt blood sugar and digestion
  • Suppress your immune system
  • Interfere with hormone balance, especially by raising testosterone and lowering progesterone

All of these can affect your skin—by triggering excess oil production, slowing down healing, and increasing breakouts.

But here’s something even more interesting: your skin has its own version of the HPA axis.

Yes, your skin cells can produce and respond to stress hormones too. When you're under stress, skin cells release CRH (corticotropin-releasing hormone), which increases oil production in your sebaceous glands. CRH also stimulates your skin to make more inflammation-related chemicals (like IL-6 and IL-11), which can make breakouts more red, swollen, and painful.

Other stress-related hormones (like ACTH and α-MSH) —also made by skin cells— can boost oil production too. And a nerve chemical called "substance P" becomes more active in acne-prone skin. It increases oil production, causes inflammation, and may even lead to larger, more painful pimples.

So when stress rises, your skin is not just reacting—it’s actively participating in the stress response. And that’s why managing cortisol isn’t just about feeling calm—it’s about helping your skin heal.

How to Know If Cortisol Is Affecting Your Skin

Here are some common signs:

  • Acne flares during or after periods of stress
  • Trouble falling asleep or waking up in the night
  • Feeling “tired but wired”
  • Mid-day energy crashes
  • Cravings for salty or sugary foods
  • Increased anxiety or irritability

If these sound familiar, your cortisol rhythm may be out of sync.

How to Lower Cortisol Naturally

The goal isn’t to eliminate cortisol—it’s essential for survival. What we want is to restore a healthy rhythm: high in the morning, gradually tapering throughout the day, and lowest at night.

Here are five evidence-informed ways to help regulate cortisol:

Prioritize Sleep

Deep, consistent sleep helps reset your HPA axis (the brain-adrenal communication system). Most adults need 7–9 hours of uninterrupted sleep each night.

Balance Your Blood Sugar

Cortisol and insulin have a close but inverse relationship, so when your blood sugar drops, cortisol production increases. To prevent these fluctuations, eating at the same time every day and balancing your meals with the proper macronutrients can help.

Try Adaptogenic Herbs

Many herbs with adaptogenic properties have been shown to support cortisol regulation and stress resilience. As always, speak with your naturopathic doctor before starting a new supplement.

Change the Intensity of Your Workouts

If you’re constantly exhausted, HIIT may be too much for your adrenals. Try walking, yoga, or strength training instead.

Nourish Your Nervous System

Daily practices like deep breathing, meditation, or grounding can help reduce sympathetic (fight-or-flight) activity and lower cortisol.

How I Help Patients in the Breakthrough Acne Program

In my Breakthrough Acne Program, I use saliva testing to look at your full cortisol curve—morning, noon, evening, and bedtime. From there, I customize a plan that includes:

  • Adjusting the timing of meals, sleep, exercise, and supplements to support your natural cortisol rhythm
  • Using targeted nutrient support
  • Setting consistent bedtimes and wake times to stabilize your sleep-wake cycle
  • Timing protein and carbohydrate intake strategically throughout the day
  • Building in restorative activities during specific windows when cortisol may be spiking or crashing

I don’t just tell you to "stress less"— I help your body recover from it.
